General Terms and Conditions
1. By signing the history sheet (electronic form), you have acknowledged all terms and conditions of the clinic.
2. Your personal and medical information is securely stored and not shared with any external agency for marketing purposes.
About Personal Information
3. It's the patients/carers/parent's responsibility to provide correct and accurate personal and health-related information about the patient to the best of their knowledge.
4. For ongoing patients: It's the patients/carers/parents' responsibility to update correct and accurate personal and health-related information about the patient to the best of there knowledge.
5. The practice can use your personal and medical information to train doctors and nurses, and whilst discussing the treatment options with internal or external practitioners such as specialists.
6. From time-to-time practice can send you the clinic's promotional offers.
7. The patient willing to access personal information such as X-rays individual files should do via written request. Once the request is received, it may take up to 7 working days for X-rays and 21 working days for the personal file to be released. Administrative fees ($50) may apply.
About the condition of Payment and Payment Plan
8. The patient should pay the treatment fees in full at the end of treatment.
9. It’s the patient’s responsibility to enquire about the cost of treatment with the dentist before the commencement of the treatment.
10. Failing to pay for the treatment will be referred to the police or debt collection agency.
11. The patient will be charged $50 cancellation fees additional in their upcoming appointment if they fail to attend their appointment or cancel or reschedule an appointment within 48 hrs of their appointment.
12. $50 additional fees apply for consultation (30 minutes), treatment plan, treatment quotations, and referral letter.
13. Patients should not assume the clinic's responsibility for Payment plan related disputes or issues as Payment plans are offered by a third-party provider.
14. It’s a patient’s responsibility to consider their financial condition or situation before applying for the payment plan.
15. Its the patient’s responsibility to read and understand the terms and conditions of the payment plans offered by external providers such as DentiCare, ZIP Money and AfterPay.
16. Patients using payment plan options have to resolve payment-related disputes such as failure to pay or extension for payment by raising directly with the appropriate payment plan providers.
17. The clinic is not liable for any financial difficulty the patient may faced which is directly or indirectly related to payment plans.
18. It’s the patient’s responsibility to inform the payment plan provider where the patient decide to cease or amend ongoing treatment.
About Appointments
19. It's a patient's responsibility to maintain their appointment, especially patients with a more extended treatment plan.
20. Patient with hospital voucher: If the patient fails to attend the appointment regularly, the clinic will send the voucher back to the Hospital. The practice is not responsible for applying the extension of the hospital vouchers.
21. It's the patient's responsibility to maintain the appointment, especially undergoing treatment that requires multiple appointments. The clinic is not liable for the failure of the treatment as a result of failing to keep the appointment.
22. The patient will be charged non refundable $100 deposit fees for the booking of long duration appointment ( For example: Whitening, Crown, Implant, RCT appointments). This fee will be used as cancellation fees if the patient failed to attended or reschedule their appointment within 48 hrs of scheduled appointment. For the patient who keeps the appointment, deposit will be used towards their cost of treatment.
Other Important Stuff
23. The dentist has all right to refuse the treatment if the patient is deemed disrespectful, aggressive and abusive, unreasonable or obnoxious towards staff or doctors.
24. The clinic does not intend to in any way or form encourages patients to make their treatment decisions based on financial gain, primarily through promotional or other offers.
25. It's the patient's responsibility to inform the doctor if they wish to have a second opinion.
26. It’s the patient’s responsibility to maintain their dental hygiene, make regular appointments for a check-up, clean, and follow the instructions provided by the practitioner.
27. The above terms and conditions are subject to change and update from time to time without notice.
